What is The ASAM Criteria^®, Fourth Edition Treatment Planning Templates

ASAM has developed guidance on how to develop a treatment plan in alignment with The ASAM Criteria Fourth Edition. The document guides clinicians in how to use The ASAM Criteria Assessments to collaboratively develop an individualized treatment plan. The document also provides a treatment planning form. This form guides clinicians to develop treatment plans that address the patient's priorities and Dimensional Drivers, as well as potential barriers to transitioning to a less intensive level of care. It is organized by problem areas and provides structured information on goals, dimensional risk ratings, objectives, and action steps. The template also captures a summary of needed services, transition planning, and safety planning. This form is free to use after accepting terms and conditions. Information provided will not be used for solicitation.
