Report a Payer/Pharmacy/Coding Issue

Report Issues Impacting the Practice of Addiction Medicine

Clinicians practicing addiction medicine may encounter various obstacles to providing treatment for addiction. Please use the form below to report these issues to ASAM that are impacting your direct patient care, including:

  • Patient access to medications for addiction treatment at the pharmacy counter;
  • Payer-specific issues like utilization management, billing/coding, or claim recoupment;
  • Federal/state regulatory concerns;
  • Administrative/paperwork issues; and
  • Other safety/clinical issues.

Please include as much detail as possible in your submission, including any documents you believe add additional context.

You will need to have the following information ready, including: 

  • Basic contact information for the clinician reporting the issue;
  • Practice type (outpatient, inpatient, etc.)
  • Details of the issue and how you would like ASAM's assistance addressing it; and
  • If reporting a pharmacy access issue:
    • The pharmacy’s address 
    • Name of the medication prescribed 
    • Any patient characteristics that underpin the prescription
    • Whether the pharmacy declined to fill or did not have the medication in stock
    • How long the patient went without the prescribed medication
    • Whether the nonfulfillment was resolved.
